Quarterly Attestation Form
For students returning to in-person learning, a parent or guardian must fill out a Quarterly Attestation Form and submit it to their teacher. The form will be kept on file in the office and will need to be updated quarterly.
By signing the form, parents/guardians are attesting they will check their child daily for all symptoms of COVID-19. They agree not to send the child to school if they have any symptoms, if they have had close contact with someone with COVID-19, or if they have been told to self-monitor, isolate, or quarantine.
