Life Insurance Employees with a minimum benefit FTE of .900 receive a $50,000 term-life policy at no cost. If you have had a change in marital status or added dependent children to your family since you began working with the district you may want to change the beneficiaries listed on your life insurance.
Voluntary life insurance is available for purchase by employees who work a minimum of 17.5 hours per week. Employees can purchase coverage up to five times their annual salary and one-half of that amount for spouses. Coverage is also available for children.
To purchase voluntary life insurance please complete the Enrollment Form above, mark the Optional Life box and indicate how much you wish to purchase. If a spouse or child is to be covered please indicate such on the lower portion of the first page of the form.
A Statement of Health is required when: An employee chooses to purchase more than $100,000 at the time of eligibility or hire, or an employee chooses to purchase or increase the amount of voluntary life after their eligibility or hire date, regardless of the amount.
Conversion to Private Policy
An employee may convert their term life coverage to a private policy when they separate employment from Everett Public Schools. MetLife is notified electronically when an employee qualifies for this conversion and will contact the employee directly. For questions regarding the rates for a private policy please call MetLife at 877-275-6387.
