Year One: 

• Confirm DDA eligibility
• Apply for SSI
• Choose a Person Centered Planning facilitator
• Begin work experience with 18-21 Transition Program

Year Two:

• Participate in Person Centered Planning
• Apply to Job Foundations program  
• Apply to DVR
• Choose an Employment Provider - they will work with you in STRIVE and at your work site
• Meet to review Job Foundations report

Year Three:

• Begin School 2 Work program - your job coach will work with you in STRIVE, at your work site and in their offices. 
• Enter Job Exploration Phase - Your job coach will help you fill out applications and support you at job interviews.
