SBHC Registration and Access to Care

Students must have a parent or guardian complete an SBHC Registration Packet to enroll in the School-Based Health Center (SBHC). Packets can be picked up at the high school office or completed online.
Once the SBHC has a completed registration packet on file, students can request an appointment by emailing SBHC@chcsno.org, calling the clinic, or completing the online request form. For urgent issues, students may also see the school nurse or come directly to the clinic’s waiting area. Clinic staff will contact the parent or guardian by phone to obtain consent.
Students enrolled in the SBHC can access confidential reproductive and mental health services in accordance with Washington state minor consent laws. These services are also available to minors at any healthcare clinic in the community.
Important Notes:

Registration is required to receive care at the SBHC and only needs to be completed once.

We encourage families to complete registration even if their student does not currently need an appointment. Having forms on file saves time and makes scheduling care easier when it is needed.
