Thank you for your interest in using the Teen Family Childcare Program at Sequoia High School  We are a small program with a caring staff of school employees and volunteer workers. We strive to create a safe, caring, comfortable, and stimulating environment for your children.

Below are the five steps that need to be completed in order for you to use the Daycare at Sequoia High School. Please note all of these items must be completed BEFORE your child can begin daycare.  The packets are available in the main office.

1.) Complete DSHS Application packet; DSHS Application, Verification of Enrollment, and DSHS Working Connections. These forms are required unless you are doing private pay and can show proof of ability to pay. Please fill out the application to the best of your ability. Prior to your first day using the daycare, the daycare staff will contact DSHS and allow you to talk to a DSHS representative.  If you qualify, this will allow you to get coverage over the phone.  Due to a change in coverage laws, we must know have a confirmation of DSHS approval before your child is allowed to start daycare.  

2.) Complete the Certificate of Immunization for your child. Your child immunizations must be current according to the Vaccines Required for Childcare sheet.

3.) Complete and sign the Child and Family Information sheet.

4.) Read, sign, and return the Daycare Center Handbook and Anti-Choking Policy.

5.) Give your class schedule to the daycare staff as soon as you receive it. We need to know where you are while your child is in our daycare.
