Evergreen Athletics

Evergreen Middle School's athletic program dynamically supports the academic mission of the school.  Everegreen's mission is to set high expectations for its coaches and student-athletes.  Our athletic program will promote interscholastic athletics that provide lifelong experiences for student-athletes while enhancing their achievement of educational goals.  We want our student-athletes to learn the values of teamwork, pride, respect, commitment, good work ethic, and sportsmanship.  It is our hope that through our athletic programs, participating student-athletes will adopt these guidelines and develop a positive winning attitude that will carry over into all aspects of their lives.

Evergreen Sports Registration

Please click here to register your athlete using FinalForms.  Registration is open for 2024-25 Fall, Winter and Spring sports.  Follow the prompts to create your account, create your student's account and sign your forms.   All student athletes will need a current physical using the approved physical form which you can upload directly into FinalForms once completed.

Evergreen Sports Physical

All students are required to have a sports physical to participate in a sport. The form needs to be filled out by a medical professional. All physicals are good for two years. Evergreen Sports Practice Schedules

All athletic practices are held Monday - Thursday from 2:10-4:15pm unless otherwise stated. An activity bus is available for all athletes at 4:30pm.  

Evergreen Sports After School Transportation 

Our athletes have the option of taking an activity bus home after practice throughout the sport season. The departure time is 4:30 pm and the bus will drop students off at the nearest bus stop to their house. It is important for parents to note that the arrival time for this bus varies depending on the number of athletes riding it home each day. Typically in the Fall sport season there are more students riding and, therefore, a longer ride-time to get home. Also, during a sport season where the number of athletes is not large, we may suspend bus operations.
