2023-2024 Immunization Requirements - Washington State requires a validated Certificate of Immunization Status (CIS).

Medication Authorization Order - For prescription and OTC medications to be kept in School Health Room, form must be completed and signed by a licensed health care professional and a parent/guardian.

Epipen Antihistamine Order - Must be signed by a licensed health care professional.

Student Technology Use Agreement - Must be signed by a parent/guardian so student can pick-up new device. 

Homecoming Dance Guest Application - Turn in the completed form with admin signatures and a copy of your guest's ID to the office
