To participate in sports at Jackson High School you must complete the following:
Physical—A current physical must be on file with the JHS athletics department. Physicals must NOT expire during the sports season. Physicals are also required for summer activities.
FinalForms—Register TODAY!
Insurance—All athletes must have insurance coverage. The JHS athletic department has insurance packets available, if your student is not covered.
Athletic Fee—A $100 athletic fee (per sport) must be paid before the first contest.
ASB Card—An ASB card must be purchased before the first contest.
**NEW** Medical Orders & Health Plans—If your student has required emergency medications at school for a severe medical condition such as asthma, anaphylaxis, seizures, and or diabetes in previous school years, WA state laws states we must have a medical order, medication, and health plan in place prior to their participation in any in-person activities, including athletics. With the start of a new athletics season, your student may need additional items completed before they are eligible to participate. Please contact your student’s school nurse with any questions. Medical/Doctor Visits
If you need to take your athlete to one of the following medical providers: a Medical Doctor (MD), Doctor of Osteopathy (DO), Advanced Registered Nurse Practitioner (ARNP), Physician's Assistant (PA), and Naturopathic Physician, a Dentist or Podiatric Physician (DPM), it is VERY, VERY, VERY IMPORTANT TO BRING A NOTE FROM YOUR DOCTOR WITH PERMISSION FOR YOUR CHILD TO RETURN TO FULL ACTIVITY. Without this, the coaches MUST hold him/her out of games and practices. The exceptions are visits to the orthodontist (unless the injury is to the mouth/teeth), dermatologist, etc. We do not need a note from a Chiropractor and your child can only be cleared by a Physical Therapist if we written permission the approved provider listed above.If you have any doubts, ask for the note – especially if they don’t say you have to rest. If they say you don’t need one, tell them the WIAA requires it. We cannot request medical information from your healthcare provider, it must come from you. Notes MUST come from where appropriate. This is a separate note from the one needed for attendance and/or PE.
