From the JHS School Nurse!
Washington state law and Everett Public Schools policy mandate that all students, with any of the following conditions; asthma, anaphylaxis, seizures, diabetes, and or any other condition requiring emergency medications at school, must have medical order, required medication, and health plan in place prior to their attendance in any school activity including athletics. If your student meets any of the listed conditions they will need the additional items completed before they are eligible to participate in athletics.
For your student to be eligible to participate in any Athletics, we will need a completed Medication Authorization Form returned, to your student’s school, by 12:00pm, noon, the week prior to the first practice. For your student's safety and in compliance with WA state laws, we appreciate your attention to this matter. Medication Authorization Forms can be found on the district website (links below) and will require your student's medical provider's signature. Some providers will provide these orders on their own forms, we will accept those too.
General Medication Authorization Order Form (Asthma & other medications)
Anaphylaxis Medication Order Form (Epinephrine for life-threatening allergy)
Diabetes Medication Order Form
Once your Medication Authorization Form is returned to your student's school, a school registered nurse will be in contact to update your student’s health plan. These forms can be dropped off at your student’s school anytime the office is open, school office hours are posted on the school’s website.
For emergency medications, please ensure your student carries required medications to all athletic activities. Coaches will be checking for compliance and will be trained of their role in the event of an emergency.Questions?
