Grade 1 to 12th :          

Kindergarten :               

• Enrollment Form (Fill out the form online; print out to take to school.)
• WA Certificate of Immunization Status (or parent/guardian may submit proof of immunizations) 
• Home Language Survey Templates (multiple languages available)

The forms below are also required for all incoming kindergarten students. If your child is enrolled in an Everett Public Schools early learning program prior to kindergarten, such as ECEAP, Transitional Kindergarten or Developmental Preschool, this information must be completed for your child to attend Everett Ready in August.
