  • Athletic Registration Process for Middle and High Schools

    We are very excited to announce that the Everett Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

    All eligibility forms must be completed in FinalForms prior to participating in any of the following programs:

    Fees
    Athletic participation fees are $100 for each high school sport and $40 for each middle school sport.  The fees help cover the cost associated with athletics including transportation, officials' fees and coaching stipends.  The athletic fee is a part of the clearance process for each season.  A student becomes eligible to compete in a contest after the fee is paid.  Fees are NOT paid until after the teams are selected.  Click here for online payments or checks can be made out to Everett School District. 
     

    Physical Forms may be printed and taken to a clinic, although if the clinic has an in-house physical form, those are acceptable as well.  The physical MUST be an athletic physical and NOT a well-child check up.
    Physical Form | This is a two-page form, please be sure to print and complete both pages of this form and have it renewed every 2 years. If the physical expiration date takes place during a season, the student must get it renewed prior to the start of the season. Failure to do so will cause athlete ineligibility and forfeiture of contests.

    Reduced Cost Physicals

    All student-athletes are required to have a sports physical every two years. For families looking for a place for an inexpensive physical, try contacting the following clinics for more information. Everett Public Schools does not endorse either clinic and provides the information as a service to families.
