Eligibility Information

  • Athletic Registration Process for Middle and High Schools

We are very excited to announce that the Everett Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

Review your eligibility status by reading the WIAA Eligibility Checklist. Additional Everett Public Schools requirements may apply (outlined in the Athletic Code below). Please contact your building athletic coordinator to have your questions answered.

All eligibility forms must be completed in FinalForms prior to participating in any of the following programs:

    High School

Winter | Basketball, Girls Bowling, Boys Swim, Wrestling
Spring | Baseball, Fastpitch, Golf, Boys Soccer, Girls Tennis, Track and Field

    Middle School
Fall | Cross Country, Football, Fastpitch
Winter I | Boys Basketball, Girls Volleyball, Boys Soccer, Girls Soccer
Winter II | Girls Basketball, Wrestling
    Spring | Track and Field

    Fees
Athletic participation fees are $100 for each high school sport and $40 for each middle school sport.  The fees help cover the cost associated with athletics including transportation, officials' fees and coaching stipends.  The athletic fee is a part of the clearance process for each season.  A student becomes eligible to compete in a contest after the fee is paid.  It is recomended that fees NOT be paid until after the teams are selected.  Click here for online payments or checks can be made out to Everett School District.

Physical Forms may be printed and taken to a clinic, although if the clinic has an in-house physical form, those are acceptable as well.  The physical MUST be an athletic physical and NOT a well-child check up.
Physical Form | This is a two-page form, please be sure to print and complete both pages of this form.  Must be renewed every 2 years. If the physical will expire during a season, the season must be renewed prior to the start of the season to avoid the ineligibility of athletes and the forfeiture of contests.
