Secondary Transition

  • Secondary transition services begin for all students who have an IEP by at least age 16 and continues until the student has graduated from high school, exited from Special Education, or reached the age of 22.

Secondary transition services focus on post-secondary goals for:
    • Employment
    • Education/training
    • Connecting with agencies
    • Independent living skills

Click here for a helpful checklist for transition services that differentiates what a student needs during his or her school years, prior to age 18, and from ages 18-22.
