• Occupational Medical Providers

You may choose your own Medical Provider for initial treatment of your work injury. However, follow-up and on-going treatment for your work injury must be from an approved Labor & Industries (L&I) Occupational Medical Provider.

You may find L&I approved Providers by visiting the L&I Medical Provider Network.

To assist employees with convenient and quality medical treatment options, below is a list of L&I approved Occupational Medical Providers in the Snohomish County area that have been identified and have partnered with the District. These Providers are designated as Centers for Occupational Health and Education (COHE) which provide integrated and managed "best practices" care.

Please note, the below-listed providers are listed for convenience purposes only. It is NOT required that you visit one of these 4 facilities.
