Whether you are experiencing a family or medical emergency, adding a new family member, scheduling a planned surgery, or experiencing another reason that has led you to take a Leave of Absence, we are here to discuss your options.
A Leave of Absence (LOA) is required any time you miss more than 5 consecutive days of work.
How to Request a Leave of Absence

Connect With the Benefits Team
- Call, email, or request an appointment with the Benefits Team at the Community Resource Center to start the process.
- Our team will walk you through the leave process, help you understand which type of leave may apply to your situation, explain your payment options while you’re away, and answer any questions you have about leaves of absence.

Submit a leave request form
- Go to Frontline Central > My Forms > Forms I can Start > Leave of Absence Request

Submit supporting documentation

Medical, Family, Military, Adoption, Childcare, and Maternity/Paternity Leave: FMLA forms from the Department of Labor are required as supporting documentation.

Student Teaching Leave: Confirmation of the dates you'll be away from your teaching program.

Need to change to your Leave?
- Should you need to change your leave dates, change the type of leave, or how you are being paid while on leave, there is a form to request those changes.
- Please be aware changes can only be made to future dates in a leave, we cannot make retroactive changes.

Submit return to work paperwork
- If you have any medical restrictions, they must be reviewed by the Benefits Team prior to your return to work.
- A doctor's note clarifying your restrictions or clearance to return without restrictions is required prior to returning to work.
Shared Leave Program

The Shared Leave program is intended to extend leave benefits to an employee who otherwise would have to take leave without pay or terminate employment with the district. Everett Public Schools does not accept donations from outside agencies (i.e. from an employee working in another school district).
Everett Public Schools established a Shared Leave Program in 1991 and District Policy and Procedure 5406 outline the parameters for implementation. The Shared Leave Program is governed by the conditions outlined in WAC 392-136A and the employee's Collective Bargaining Agreement (CBA).
In order to be eligible to receive shared leave donations:
- The employee must be on an approved Leave of Absence
- The leave of absence must be for employee, relative, or household member suffering from an illness, injury, impairment, or physical or mental condition which is of an extraordinary or severe nature that prevents the employee, relative, or household member from working; employee has been called to service in the uniformed services; employee is a current member of the uniformed services or veteran and needs to attend medical appointments for a service connected injury or disability; employee is a victim of domestic violence, sexual assault or stalking; employee is temporarily disabled because of pregnancy or needs time for parental leave
- Employee has abided by the district's sick leave or military leave policies and has depleted or will shortly deplete their annual leave (vacation) and/or sick leave reserves
- Employee has not received more than 522 days of shared leave during their total employment by the state of Washington or any state agencies, including employment by other school districts within the state
- The condition may soon cause employee to go on leave without pay or to terminate district employment
- Employee has provided the shared leave medical documentation form signed by a licensed physician or authorized health care practitioner verifying the extraordinary or severe health condition that prevents employee or relative or household member from working and expected duration of the condition, or a pregnancy-related medical condition or miscarriage. This documentation is not required for parental, domestic violence, or uniformed service leave

Do I have to fill out leave paperwork if I have enough sick leave to cover my time off?
- A Leave of Absence is required any time you miss more than 5 consecutive days of work.
- Other documentation may also be required if you have medical restrictions that need to be reviewed.
- Please reach out to the Benefits Team to confirm if your situation requires a Leave of Absence.

How long will it take for my leave to be approved?
- To help us plan and ensure a smooth process, leave requests must be submitted at least 30 days before your leave begins. If the need for leave was not foreseeable—such as in the case of an emergency—we understand and will work with you to support your request.
- Getting the required supporting documentation for a leave of absence can take time, depending on the medical provider, so we recommend reaching out as soon as you know you may need a leave of absence.
- Once the Leave request and supporting documentation have been submitted to the Benefits Team, we will review it promptly.

What is the difference between FMLA and PFML?
- FMLA is a federal law providing unpaid, job-protected leave for eligible employees
